Heart Failure Specialist Nurse Service

The Community Heart Failure Nurse supports patients to self-manage their condition, ensuring that patients receive evidence-based treatments in line with national heart failure guidelines.

The aim of the service is to offer advice and education to help patients better understand their condition, not only to improve the patients’ health and quality of life but also enable them to identify when their condition is changing.

Adults with a diagnosis of Left Ventricular Systolic Dysfunction.

Clinics are held in various locations across Trafford and home visits are offered to patients that are housebound.

Your GP or Hospital Cardiology team can refer you to this service, please contact them to request a referral if you need some help and support with your condition.
