Rapid Response Urgent Care Therapy Team

The Urgent Care Therapy Team receives referrals for patients living at home who have experienced a rapid onset of deterioration in their functional ability.

Our aim is to reduce imminent patient injury, carer breakdown or a potential admission to hospital.

The team are happy to receive referrals from any professional person and aim to assess these patients within 1-3 working days dependant upon the need identified.

A therapy assessment will take place at the patients home by a qualified occupational therapist and/or physiotherapist to identify patients needs.
The team consists of occupational therapists, physiotherapist, patient support worker and assistant practitioners.

Monday to Friday, 7:30am to 5:30pm.

The team are part of the Community Enhanced Care Service and work closely with the community matrons.
