Children’s Palliative Care Nurse

As part of the Trafford Children’s Community Nursing Team (CCNT), the service offers palliative care support to babies, children and young people who reside in the Trafford area. The Children’s Palliative Care Nurse, supports children and young people living with a life limiting or threatening condition and their families.

Support can start at diagnosis and may continue throughout the child’s life, during end of life care and beyond into bereavement support for the family.

The service is part of the complex needs service and works in partnership with a variety of services to promote and enable all children and young people to achieve their best possible outcome.

The service provides care for the children and young people (CYP) residing in the Trafford area, up to the age of 16 years, or the 19th birthday in the case of CYP with complex and additional health needs.

Consideration will be made for young people with a new diagnosis requiring end of life care to be supported by the service over 16 years of age.

  • Offering practical advice and support including referral to other services
  • Supporting families dealing with diagnosis of new or worsening condition
  • Supporting families to talk to their child, siblings, and other family members about what is happening
  • Acting as a single point of contact for families to facilitate effective co-ordination of services
  • Supporting families to develop Advance Care Plans that reflect their wishes
  • End of life care planning and provision, in conjunction with the CCNT
  • Management of symptoms such as pain, nausea and vomiting or agitation during end of life
  • Support complex discharge planning and in reaching into hospital settings
  • Facilitate rapid discharge where necessary

Monday to Friday, 4:30pm – 10pm.
Weekends and Bank Holidays, 08:30am to 10pm.

How to refer

Referrals to the children’s palliative care service can be made via telephone, email or letter and are accepted from all agencies working with children and young people. Requests for rapid discharge from hospital for end of life care must be discussed with the Children’s Palliative Care Nurse or the CCNT Team Leader before agreeing with parents / carers.
