We may be able to offer an NHS continuing care package for children or young people with health needs arising from disability, accident or illness that cannot be met by existing universal / specialist services alone.
Children and young people (0-17) who have complex health needs may require additional support that cannot be met by existing NHS services. This additional health support is assessed and thereafter agreed with an NHS Continuing Care Assessment process, then delivered through a tailor-made commissioned package of care.
