Trafford Macmillan Supportive and Palliative Care Community Team

Palliative care is care given to people with chronic, often life-limiting illness.

The focus of the Trafford Macmillan Supportive and Palliative Care Community Team is on improving quality of life. Our borough wide service provides seamless care and is delivered by Trafford Local Care Organisation staff in partnership with Macmillan Cancer Support.

Our aim is to improve the quality of life for patients with complex palliative and terminal care needs, and support their carers.

Our service is usually for people whose illness means they are thought to be in the last year of life. However, we also provide care for people with longer life-limiting illness. We also provide support to families and carers. This is a really important part of supporting patients.

The service operates from the Macmillan Wellbeing Centre and works across the borough of Trafford.

The teams provide support for adults facing life-limiting illness who are registered with a Trafford GP. The most common example is cancer, but our teams support patients with other conditions. We visit patients in their own home or care home. We also provide telephone support and advice when required.

The teams recognise that emotional, family, financial and spiritual concerns may be just as important to you as physical problems. We can:

  • Provide treatment and advice to help manage any symptoms
  • Offer practical advice and support to do the things that are important to you
  • Support families and carers
  • Signpost to other services including financial advice
  • Support you to make choices and plan for your future

Patients will see a Macmillan professional team member with specialist training. In the team there are a number of professionals who work together as one team.

  • Occupational Therapists
  • Physiotherapists
  • Family Support Worker
  • Medical Consultants
  • Assistant Practitioners
  • Support staff

Our teams work closely with GPs, District Nurses, urgent response teams, hospices and hospital teams. Our aim is to make your care as easy as possible and we do this by coordinating the service you will receive with other professionals.

We take referrals from healthcare professionals. Your GP, District Nurse, Hospital Doctor or Nurse can refer you to our community service.

The service operates from 9am – 5pm, Monday through to Friday. The service is closed on weekends and bank holidays.
