District Nursing Services

District nurses assess people and provide nursing care for patients in their home or in their normal place of residence, as well as in other community settings.

We aim to make a difference everyday to the lives of the people we visit and can provide increasingly complex care for patients and support for family members. We provide care for people aged 18 and over.

We aim to provide an easily accessible, responsive service which meets the identified nursing needs of our patients and their carers. We try to ensure that patients are able to maximise their independence at home rather than need hospital care whenever possible.

We aim to provide care to patients in their own home when they are unable to access a treatment room, or when the nursing care they need is best delivered in their home.

 

We provide a wide range of nursing care packages personalised to meet the health needs of each individual and their carers and family.

Conditions that we provide care for include:

  • Wound care
  • Pressure area and skin care
  • Catheter management
  • Bowel management
  • Injectable medicines
  • Eye care (post-op)
  • Vac therapy
  • Drain management
  • Diabetes care and management
  • Health promotion
  • Palliative (end of life) care.

The District Nursing Service places the patient at the centre of the care planning and decision making process using a single assessment process.

We work in close partnership with GP practices, secondary care, community services, the Local Authority and others to meet the nursing needs of adults in Trafford. We also provide educational placements for other health care professionals, including both pre and post registration nurses.

Care packages include:

  • Nursing assessments, for CHC, funded nursing care or residential care
  • Medication administration
  • Acute (post-operative) care
  • Diabetes care
  • Urological conditions, including catheter care, continence promotion.
  • Gastrointestinal problems e.g. support with stoma care, gastrostomy feeding regimes and medications
  • Palliative care and symptom control.

Although our care is delivered in homes and other community settings, our district nursing team have bases at the following locations across Trafford:
