This service offers patients with a confirmed diagnosis of Parkinson’s, parkinsonism or a Parkinson’s plus condition with care and support, as well as clinical monitoring, medication advice and referrals where appropriate.
Our aim is to prevent hospital admissions, promote early discharge and reduce time needed by consultants to review patients.
