We have introduced new contact details for the Community Single Point of Access (SPoA) for Adult Community Health Services across Manchester and Trafford.
The SPoA is the main point of contact for referrals, appointment enquiries and questions about accessing services. By bringing a number of existing contact routes together, we’re making it easier for patients, carers, health and care professionals, colleagues and partner organisations to get in touch, helping ensure referrals and enquiries reach the right team more quickly.
The new contact details are available now. Existing contact routes will remain in place until Saturday 1 August 2026 to support the transition. After this date, previous SPoA phone numbers and email addresses will no longer be in use.

About our Adult Community Health Services
Our Adult Community Health Services provide a range of care and support in community settings, including people’s homes and local clinics.
Services include district nursing, physiotherapy and occupational therapy, alongside a range of other services that help people stay well, maintain their independence and receive care closer to home.

Yes. The SPoA does not currently cover:
- Learning Disability Services
- Sickle Cell and Thalassaemia Services
- AAA Screening
- Discharge to Assess (D2A)
- Community Dental Services
- Expert Patient Programme

The existing SPoA contact routes will remain available until Saturday 1 August 2026.
After that date, referrals, appointment queries and questions about accessing services should be made using the contact details on this page.
Referrals submitted before Saturday 1 August 2026 will continue to be processed as normal. There is no need to submit a referral again.

The MFT Urgent Care Single Point of Access is a separate service for urgent clinical referrals, triage and advice.

The service provides rapid clinical assessment by acute consultants and community advanced clinical practitioners.
Following triage, patients may be referred to services including:
- Same Day Emergency Care (SDEC)

- Community Crisis Response
- Hospital at Home
- Community IV Team
